Dive was supposed to be 80' max for 35 minutes plus then an extended saftey stop. Diver in question didn't pay close enough attention to depth and maxed out at 96', ran a sawtooth profile (hit 90+ three or four times during the dive), didn't realize they were in deco when ascending to the safety stop, didn't vent enough air on the BC and couldn't hold depth, and surfaced still in deco (aka "omitted decompression").
