IMPO, only two valid reasons for ANY diver to run out of air: 1. brain dead stupidity, and 2. a death wish.
My two rules for never having experienced OOA, gained from 41 years of active diving, 3,800 dives world-wide: 1. Pack a minimum of 50% more gas than you know you're going to use, (i.e. bigger tank and pony), and 2. dive ONLY with the buddy-between-your-ears.
And one other solution I grudgingly acknowledge and accept: take a DIR Fundamentals course from MHK and dive DIR.
