This is a terrible tragedy. I think we should all try to remember that Scuba is an exreme sport; the moment we leave the surface, we're on life-support equipment. Regulators, BCD's, and SPG's should be cleaned and checked regularly. At depth, every effort should be made to avoid panic, probably the greatest threat that divers face. This is a matter of skills practice.
