Confirmed it was the Dream though I agree with Justfer that it was pilot AND BUDDY error. His 2 buddies reportedly watched him surface and then left him to continue their dive. They didn't take their ill friend back to the boat. Apparently he never inflated his BC nor ditched his weights.
