I have to wonder what the real story is. Was the tank over pressured or was there some other source fo the failure? No matter how you look at it, we need to be very careful with compressed gasses of any blend.
I do not expect the newpaper reporter to take the time to report the actual cause of this accident. If they did, most readers would not read it anyway. Interest in cylinder safety exists in a small, but wonderful community.
