There is significance in the distinction between doing the
dive solo versus having your buddy disappear on you. In the
latter, you're doing an unplanned solo segment, and you need
to look for your buddy in case he is in trouble.

My dive plan is different when I'm alone versus with someone.
I suspect your plan is dramatically different from when
you dive with Terry or Troy versus your trip to Wisconsin.

On a tangent to your reference to gun accidents: You're
a stroke at the MJB standard if you point a gun at
someone and pull the trigger when you don't intend to kill
them. In my mind, that always is an intentional shooting
because the basic rules of firearms are:
1) never assume it's unloaded (check!)
2) never point at people
3) never pull trigger while pointed at anything you don't mind seeing shot

It's definitely an attitude issue, much as you've referred to for divers.
Someone pointing one at me may get a rather forceful response with
a gun I KNOW is loaded.
