A buddy would have had a chance to save him or at least drag him to the entrance where recesitation could have been performed. He was reasonably close to the entrance.
The one undeniable event that could have averted this tragedy would have been cave training. Although he was highly trained in many facets of diving and rescue, he dove past the limits of his training by entering that cave. Such a waste.
