I don't know for sure, they were not my tanks. But
why would two tanks exhibit the same problem after
both being vip'd and filled only two days earlier.
Besides, the description I heard was that the valves
were found loose threaded on the tank with the
o-rings just lightly sealed. We are talking about
the valve itself being loose from the tank, not
a slightly cracked open valve. The o-ring in
discussion was the ring between the valve and
the tank, not the o-ring that seals between the
reg and valve face. Perhaps I should have made
that clearer.

As I understood the scenario, both tanks were placed
into the diver's RIB after filling (I believe he
trailered the boat to Sport Chalet after prepping
the boat somewhere else, like a car wash maybe?).
It sounds like they were not touched after they
were put on until he was ready to gear up for the
dive.

But even then, how many people can right now go to
their tanks and turn the valve to remove it from
the tank itself without moderate strain or smacking
the valve handle with a rubber mallet or the palm
of a sturdy hand to loosen the seal?
