I don't know of such a site on the web, but DAN has published annual accident summaries for a while. They probably have back issues available.
Your biggest killers are drowning and lung overexpansion injuries. IMO both result most often from panic, thru spitting the regulator in the former and holding your breath in the latter. Almost every other significant threat results on a chamber ride or hospital stay - but these two will put you under.
