I can't speak for others, but at the shop I DM for, we have students completely remove/replace gear at the bottom of the deep end of the pool. Every one is too worried about embolisms to have students surface without the reg in their mouths.
My mother told me that she had to ditch and don in open water off L.A. in the 60's.
