Put the boot on a hard floor (like concrete) then take the tank and "lightly slam" the tank into the boot. Then pick up the whole thing and tap the boot on all the way against the floor.
This seems to be the best method to put them on.
To remove a boot I use a large heavy rubber mallet, and tap the boot off the end of a tank.
