Facts as known:
Diver surfaces low or OOA and grabs another tank from their boat in order to decend and complete a deco obligation. It was their second deep dive of the day. Second tank floats to the surface, diver found dead at 90 fsw.
Questions for analysis:
So why would someone with over 3,000 dives incur a deco obligation without adequate gas supplies? Poor planning, no planning, narc'd to hell, what?
Where were the others in her team? If they were all diving similar profiles didn't they too have a deco obligation? Were any of them also low on air? Didn't they think that having to take another tank back down was serious enough to warrant an escort?
