Unless you are narc'd to hell as you put it, a person has no business planning a dive where they need to surface for another tank to complete deco nor should you screw up and suddenly realize "oh I need deco time and I'm out of air". Plan your dive intelligently, dive your plan intelligently, period! Nobody has any business sucking a tank dry unless there is a gear malfunction. No offense meant to the deceased or her family but I just don't understand how people get in situations like this.
