The newspaper accounts indicate that something unusual must have happened here. I concurr that we should not jump to conclusions until we know more about what happened. But how can we do that. Follow-up articles tend to be rare. Is there an "Incident report" or something similar that will be made publically available so that we can learn what happened and learn from it?
This death is tragic and, to the extent possible, IMHO learning from it is one of the only positives that can come of it.
