Was this a planned deco dive? That wasn't clear to me. In fact, did Mia descend to complete mandatory decompression or some other reason-like she wasn't feeling well and wanted to hang longer.
I think you hit the nail on the head with your first post-bad things happen to people regardless of experience when they disregard really basic principles like gas management and diving in teams. While details are lacking, it sure sounds like at least those problems occured.
