You're right. While I don't have the facts, it may have been that she grabbed her tank from the first dive of the day which could have been nearly empty. If, as Eric suggests, she may have lost track of time and burned her first tank, she may also have had to make a rapid ascent and sustained a cerebral embolism which manifested moments later on the hang causing her to lose conciousness. Without a buddy to look over her...
With few facts, I am of course speculating. The scenario does fit those few facts, however.
