I have read of this happening twice. I am sure it has happened more, but that is two I know of for sure. In both of these cases the DM imediately dropped below his/her (one of each) MOD and grabbed the (newbie) diver that was too deep and signalled they should go up. In both cases the DM was much more upset than the (oblivious) newbie diver. If you look at the actual data on toxic reactions you see that MOD should really be called the ASOD (always safe operating depth). The actual MOD is much deeper for most people. Note I am not advocating breaking the MOD. But a short spike in PPO2 to say 2-2.5 is very unlikely to cause a toxic reaction.
