I just got off the phone with an old dive buddy that survived a lung overexpansion injury. He was shooting photographs at Catalina in the early 80's and doesn't remember ascending at all while holding his breath (though he clearly must have ascended in some sense.) The interesting thing is that he felt no pain/had no indication of injury until feeling shortness of breath on the surface. In other words, you don't get any warning for this one.
