You do have to use a catheter. If I'm diving under an hour I don't bother dealing with it, if my profile is over 1 hour then it's worth the minor inconvience..
There is nothing worse than being 10' from the boat and having to pee like a race horse and then realize that you've got another half hour of deco left ;-)
Most women that I know use diapers or depends..
