If this is the most likely cause of failure then it sounds like the operator is more at fault than the equipment. Some people want to torque everything down like it's a bloody pinon nut. I've seen people do it with yoke valves.

There's no excuse for having bad o-rings. They are cheap, change them often--before they even look damaged.
