Examples of divers barred at the dock?

It's been suggested that it is understood to be the job of boats, stores, and DM's to screen for diver skill levels. I wouldn't have thought so, but maybe my experience is too limited.

If one assumes that five percent of divers have excessively poor skills, then on average one diver would be left at the dock for every boatload of twenty divers.

Anyone have examples of local divers barred* at the dock (or earlier) based on screening for specific diving skill levels? What type of screening was used?

*Barred, not just noted by the DM as deserving of additional scrutiny once diving commences.
