I think it was a vestibular hit. I emailed him your list of symptoms and he basically had them all, if in some minor form. He said the next day he still felt woozy if he moved his head fast - sounds inner ear related to me.
Very scary. Shallow dive, very experienced and healthy diver, no fast depth changes, and he certainly could have died if alone.
