Cygal's predicament could have easily been avoided, had she stuck to the limits of her certification. She willfully determined, prior to jumping into the water, to exceed the limits of her PADI Open Water certification. She knew she would exceed her 60' depth limit, and at 110' chose to leave open water and enter an overhead environment.
So many accidents in DAN's annual accident review share the common thread of exceeding certified limits. Are instructors not informing their students of the limits of their certifications? Or are instructors telling their students they are certified for more that the really are? Or are so many people careless risk takers?
