The DIN connection was not screwed in tight.

The regulator was pressurized and then turned off.

Through lowering, movement, leak or any of a number of possibilities the 2nd stage started bubbling and zeroed the pressure in the reg.

Then the movement of the boat ratcheted the connection off the valve.
