I have it on pretty solid information that they way I described it is what has happened, however, I was just trying to see if anyone had added information..

This is one example of the reason why I have been saying very recently that teaching students OOA drills while sitting on their knees is completely worthless because it eliminates the very real example of how you need to orally inflate your BC, and it happens very often that the donating diver over inflates his BC, to compensate for the OOA diver, and then all that needs to happens is that the donor gets too seperated from the OOA diver, then the donor goes up and the OOA diver sinks...

Admittedly I'm speculating here, but by all accounts this appears to be what happened...

During the GUE classes that we have been talking about lately, we do in fact require that students practice OOA ascents for this very reason...
