failure to plan your dive properly,
failure to monitor your SPG continuously,
failure to stick to your dive plan if any,
failure to bring along adequate sized tanks for your planned depth,
failure to have redundancy in your air supply such as a pony or a slung bottle turned off but ready to go,
failure to allow an adequate reserve of air at the end of your dive,
failure of your SPG on the low end of its range to function properly,
unfamiliar or extreme narcosis and inability to concentrate on your dive procedures,
failure of your buddy system.
