Just a small point of reference, it's a PFO or Patent Foramen Ovale.. ANd generally you do need to specifically need to request it and it generally does not form the basic part of a physical exam.
When I had my done I had my doctor recommend it and I was therefore able to have it covered by insurance. If memory serves me correctly it was in the neighborhood of $700...
Your doctor should know what this is and you should let him know that your concern is because of scuba diving, because it's estimated that 25% of people have a PFO, but it isn't anything to worry about unless you are on scuba..
