About as common as a DIR Unified Dive Team ending up in a real OOA ;-) (sorry, couldn't help myself)...
But even in my limited diving career, I've had my second stage fail open (Poseiden. Woosh, there goes the air!), and I had a buddy have the tank o-ring fail catastrophically (in Lake Tahoe, of all places. not where you want to do an ESA). It happens. Of course, gear selection has a lot to do with it (bye, bye, Poseiden)...
