14 open water students to one instructor is against the Maximum ratio of students to instructors. Also using nitrox ..ie. rebreather is also a standards violation. Pacific Star on that date was a Scuba Schools of America Charter so the instructors were from the Montclair shop. Who is directly attached to the Woodland Hills Facility. Trained with the lack of following and knowing basic standards of teaching scuba....Accident ready to happen!!!! AND IT DID...
