Know one is blaming anyone. The basic problem is lack of proper training and protocal in teaching people how to dive. This is a good lesson for everyone. An inquery by the certifing agency may help strenthen it's training prodcedures to help preclude this type of accident to ever happen again. Accidents just don't happen in most cases they are caused by outside influences.
