The travel MDs I've consulted have recommended getting HEP-A and HEP-B vaccinations before traveling just about anywhere these days. Depending on what part of the country, and what kind of accommodations, you will be spending your time in Typhus and Tetanus boosters might be good idea.

Anti-malarial concerns are minimal in the Cayes, and even though some sources recommend prophylactic measures for inland areas of the country, up to now we have never taken anything when heading to that part of the world, even when spending time near the Guatemala / Belize border. Quite frankly, we've found the side-effects of Lariam and some of the other anti-malarials to be rather disconcerting, particularly if it's a dive trip.
