A responsible DM should have questioned the diver as to why he surfaced with an empty tank. Didn't anyone give a rat's you-know-what, or just "took for granted" it was bad gas management? There just might have been an acceptable reason. Or are we as divers always guilty of breaking the "rules" unless proven innocent?
