Mike I was thinking about the 2nd stage leak. One other thing it could have been was the vivi knob on the side of the month piece. I have seen those leak, they have an o-ring that is almost never changed on overhaul.
I have seen water coming back through the first stage, but that was because of a flooded tank, the diver was spiting up red rust for a couple of hours after the dive.
