I replace hoses when they have any signs of damage or irregularity. I had one regulator hose develop a bulge. I have gouged and scraped hoses while diving.
I have had a high pressure hose start to leak during a dive that was the only one that ever failed in use. The high-pressure hose, especially if you use a HP tank is likely to fail with time. I replace my HP hoses every time I do a regulator rebuild if they are old. For the most part I ding them up bug hunting and replace them because of that.
