An investigation into the death of a veteran local scuba diver has concluded that he ignored basic safety procedures while exploring a sunken ship.

Steven Donathan, 49, reportedly put his life in jeopardy on June 25 when he didn't mark his route with a guide line, then abandoned his diving partner to enter the ship's boiler room which had been off limits to divers.

The breaches of safety protocol are explained in a 20-page death investigation report prepared by San Diego police in conjunction with lifeguard divers.

The investigation concluded that Donathan was solely responsible for the accident.

His was the first death inside the Yukon since the ship became an underwater attraction five years ago. He was reportedly training a student during the fatal dive.
