The common thread with most accidents is that a series of mistakes compounded on each other. There are the exceptions medical problems and boat accidents, but objectively I think most would agree that a series of mistakes usually occurs. We all have heard our own "versions" of accidents. I usually make a mental check against what I am doing or how my gear is configured. I think the detailed comments should be left to the experts. Respect for the victim's families should be observed.
