I like your equation, but the first part about consequence I'm not sure of. The overall equation sort of imples a result of 0 or 1. You live or die. You are trained at depth or other risk factor just to change your response to something Going Wrong. Your dive procedures are as good as they are going to be. What changes is your response when something actually goes wrong. Shallow, you do one thing, deep another, but before something goes wrong, you make no adjustments.
It's just a theory, a logical extension of some data.
