I have a 5' hose bungeed under my chin. I've never had any problem making surf entries/exits with it and I've never had it inadvertently come undone.

The 5' hose is shorter than what cave or wreck divers use - normally around 7'. However, for open water use, it's worked well. In fact, a few weeks ago while I was diving in North Carolina, my buddy suddenly ran out of air (long story for another time) and pulled the reg right out of my mouth. At the time I was facing away from him trying to take a photo of a jellyfish as we did our safety stop, so it was a complete surprise - I never saw it coming. However, the hose slipped right over my head without snags or tangles and I was able to put the bungeed octo in my mouth without losing a breath.

So in my opinion having a bungeed octo is a reliable system that works well for open water recreational diving.
