I got my first punctures in Cocos Island last week, pretty bad, about 7 spines. The one sticking out we got, the rest a friend who had them in Lembeh gave me good advice. Take a brush handle and whack on the area for 10 minutes or so, skin should be pretty red and raw (anmd a little blood) by the tie your done.

Next day, could hardly see anything, the beating apparently breaks up the spines and they are quickly dissolved by the body

But I'm not a doc and that's not medical advice. We were 350 miles off the coast so little choice. The DM said htey always just leave them in and alone.
