The three biggest causes of getting lost that I see:

1. Failure to eyeball where the anchor is at the beginning of the
dive.
2. Failure to note the depth of the anchor and use the depth gauge
as a nav tool.
3. Failure to look at the compass now and then and grock which way
is North
