Cool find, Merry! I remember Atlantic City in the 60's - we'd find fragments of glass bottles that had been dumped at sea - polished and ground by the waves. Maybe there are a couple more laying in Neptune's chest?

Salt Lake is the bottler's location. For teh manufacturer, look for the bottle manufacturer's stamp; here are some samples for several supplying bottlers in the western US in this link.
