The Kirby Morgan helmets I've seen have a nose block that can be used to help equalize (though the Frenzel maneuver or swallowing can also work well). Its a lever you turn that positions a tab against which you can press your nostrils, then moves out of the way. I tried one out a few years back and didn't need to use the block.

The Mark V helmets also offered a nose block attachment that I believe were mounted just under teh front face palte allowing the diver to press theri nostrils against it for a Valsalva method clearing.

Not sure about old commercial helmets, but am sure Nick Icorn would know - he'll likely be at Scuba Show. My guess would be they'd yawn or swallow hard ot equalize (or just blow out their ear drums).
