It was an air spool failure. There are two small o-rings that need to be replaced and the mating surfaces need to be cleaned.
Any competent dive shop (which are getting harder to find) can fix this for you.
I'm surprised it lasted 15 years. I have to replace mine 2-3 times a year. However I dive frequently.
