According to the reports, when their air was low, the signal to surface was given. Mia decided she wanted to finish the count she was doing. This caused her decompression and AAO situtation, which her "buddies" didn't face. everyone else surfaced with no problem. She didn't miss the safety stop, just didn't have enough air to finish it. When she surfaced, the report said she told them her computer indicated she needed more deco time. She asked for another tank to finish it. Could she have done the deco on O2 at the surface? No way we will ever know. Much too sad.

The basic points are, She and her buddies didn't dive the plan, They broke the buddy system several times, and she went back down after making it safely back to the surface. Most likely any break in this chain of events, and Mia would still be counting fish and plants. This last is MHO, not fact.
