JR -- your points are well taken but I'm wondering why you state that this couldn't have happened on these other boats?
What's to stop a diver from forgetting to tag their tank for a fill, assuming it's been filled, grabbing it without checking the PSI and jumping in the water? I don't remember anyone checking my gauges before I dived from either the Peace or GE, or any other boat.
This is the kind of basic stuff that divers have to be responsible for themselves.
