I remember reading somewhere that sharks have amazing powers of recovery. They are all but immune from cancer and are extremely resistant to infection. This is why there is so much medical research into shark physiology.
Assuming the spear didn't damage a major organ or blood vessel, I would guess that the shark would survive. The spear will fall out over time and, being cold blooded, the shark will be able to survive for an extended period of time without feeding.
