Interesting thread, one time I was training in a twelve foot tank and I got super vertigo for a few seconds probably caused by a block. Another cause is cold water in the ear canal - instant and sever vertigo is the result for some, but since he was confused he may have had a touch of hypoglycemia, or simply not enough calories that day. I would have him get an ekg just for kicks and the DAN suggestion was great.
