I am not saying it doesn't occurr but I AAM saying I beleive you can prevent that sort of thing, Tourniquet Tight is NOT neccessary.
There are a LOT of depth compensating options including the mounting of a pad with straps to the Dry/Wet-suit.
My own physical shape allows for a not so tight fit at the wrist with depth compensating bands. Some people have wrists nearly a large as their hands so special adjustments have to be made but this is not common. Even at depth I have yet to see one slip over the hand even if somewhat loose (not dangling), broken bands, bad buckles YES but not slipoff
