We aren't killing them, they are doing it to themselves. They are totally responsible for their own fitness level, training, equipment and decision making on how to bring it all together to sucessfully conduct dives. You're right about an advance dive plan. This guy might be alive today had he dropped his weights and started kicking up as soon as he realized the buddy air sharing wasn't going to well. That simple skill that every agency teach, divers just don't get it. Most dive briefing from DM's have this aspect built into it, "drop your weights if you are in trouble." Some shops will even give you new weights if you have this need to ditch and are worried about the money.

I can appreciate your concern as Jeff Bozanic stated in his work DAN is compiling the data on recreational dives. Maybe you can get something from them.
