It not the academics that fail most (although a few have failed imho due to laziness) it is the skills and not that people get 'one chance and only one chance'. The implementation of the skills in a realistic setting (that one has already been described by mike) is what taxes many beyond their comfort zones and is grounds for failure. Who do you want to dive with? Someone who can calmly handle the requsite emergency or someone who has demonstrated their lack of ability?...or better yet, someone who hasn't even tried?
